About Us

Unite South Carolina is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. The network is supported by a South Carolina-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Community Roots

Our community champions include The Navigation Center, Just Say Something, Umoja Village, Palmetto Project, United Way of Allendale, Barnwell & Bamberg Counties, and the Carolina Teen Center.

Unite South Carolina serves partners in Greenville, Anderson, Oconee, Pickens, Greenwood, Newberry, Laurens, York, Fairfield, Edgefield, McCormick, Abbeville, Richland, Lexington, Calhoun, Orangeburg, Bamberg, Barnwell, Allendale, Hampton, Jasper, Beaufort, Charleston, Berkeley, Dorchester, Saluda, Aiken, and Colleton counties.

Network partners are now able to send and receive secure, electronic referrals in Delaware, Georgia, Maryland, Ohio, South Carolina, Tennessee, Virginia, West Virginia, and the DC area through the Unite Us Platform, which ensures communities and individuals can seamlessly access much-needed resources and services across state lines.

Just Say Something is pleased to be a part of launching the Unite South Carolina network! We know Unite Us will be a great resource as we strive to provide a continuum of care for those we serve in the community.

Any service organization or entity who is truly serious about providing the best care to its clients desires to be effective. The partnership between Umoja Village and Unite Us enables our organization to expeditiously connect those we serve with the resources they need. Umoja means unity. In unity, we are making positive differences in the lives of others. What better way to demonstrate that love is an action word.

Palmetto Project is proud to join together with Unite Us to streamline the secure referral process between organizations and agencies that work collaboratively to build successful communities in South Carolina

It is such a pleasure as a rural community to be included early in the engagement process for Unite Us. This platform will be a welcomed and very instrumental asset. It will allow our full-time staff, community partners who don’t have websites, and other resources to all interconnect the needs of our community members to direct solutions! The ability to build the platform's participating partners to meet the dynamics and uniqueness of our area, free of charge, will be of immeasurable impact for generations to come!

Carolina Teen Center is thrilled to partner with Unite Us because it is allows community-based businesses to network, and send referrals to organizations on the Unite Us software.

We have a heart for people and want to get them connected to whatever resources they need. Oftentimes people call in and need help with finding a home, a job, rent, or utilities assistance. This system allows us to link them with an agency that can help them. One thing we love about it is that we can go in and see the timeline of the referral. Before, The Meeting Place Church of Greater Columbia would often wonder if that person got help, but with this particular system through Unite Us, we're able to see the timeline. We love that it closes the loops, and even the client gets to know the status quickly. This is a game changer.
