About Us

Unite South Carolina is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. The network is supported by a South Carolina-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Community Roots

Our community champions include The Navigation Center, Just Say Something, Umoja Village, Palmetto Project, United Way of Allendale, Barnwell & Bamberg Counties, and the Carolina Teen Center.

Unite South Carolina serves partners in Greenville, Anderson, Oconee, Pickens, Greenwood, Newberry, Laurens, York, Fairfield, Edgefield, McCormick, Abbeville, Richland, Lexington, Calhoun, Orangeburg, Bamberg, Barnwell, Allendale, Hampton, Jasper, Beaufort, Charleston, Berkeley, Dorchester, Saluda, Aiken, and Colleton counties.

Network partners are now able to send and receive secure, electronic referrals in Delaware, Georgia, Maryland, Ohio, South Carolina, Tennessee, Virginia, West Virginia, and the DC area through the Unite Us Platform, which ensures communities and individuals can seamlessly access much-needed resources and services across state lines.

Carolina Teen Center is thrilled to partner with Unite Us because it is allows community-based businesses to network, and send referrals to organizations on the Unite Us software.

We have a heart for people and want to get them connected to whatever resources they need. Oftentimes people call in and need help with finding a home, a job, rent, or utilities assistance. This system allows us to link them with an agency that can help them. One thing we love about it is that we can go in and see the timeline of the referral. Before, The Meeting Place Church of Greater Columbia would often wonder if that person got help, but with this particular system through Unite Us, we're able to see the timeline. We love that it closes the loops, and even the client gets to know the status quickly. This is a game changer.
